Who Can Refer

Anyone Can Make a Referral

We welcome referrals from all sources. You don’t need to be a professional or have everything figured out before you reach out. If you know someone who could benefit from our support, that’s enough to get started.

Support Coordinators

Refer participants on their NDIS plan who need a registered provider for funded supports. We work collaboratively with coordinators throughout the process and keep you informed every step of the way.

Family Members & Carers

You know your loved one better than anyone. If you think they’d benefit from our support, reach out and we’ll have an honest conversation about what’s possible and how we can help.

Allied Health Professionals

OTs, physiotherapists, social workers, GPs, and other allied health professionals are welcome to refer patients to complement their existing care plan. We work alongside your team to make sure support is consistent and coordinated.

Self-Referral

If you’re an NDIS participant, you can refer yourself directly. No coordinator needed to get started. Just get in touch and we’ll take it from there.

How a Referral Works

We’ve made the process as simple and stress-free as possible. From first contact to active support, our team handles the details so you don’t have to.

We Review & Make Contact

Our intake team reviews every referral promptly and reaches out to the participant or family within one to two business days to acknowledge receipt and outline next steps.

Needs Assessment

We have a friendly, no-obligation conversation with the participant to understand their goals, preferences, and support requirements. No paperwork. No pressure.

Support Begins

We match the participant with the right support team and get services underway at a pace that feels comfortable for everyone involved.
